   for non-Arizona teachers

2. Application for Admission as a Graduate Non-degree Student*

3. A check for $25* made out to the University of Arizona

*You may omit these items if you are currently enrolled at the University of Arizona (i.e., you took a UA class in Spring or Summer 2008).

Please note: you do NOT have to submit the Domicile Form and the Immunization form.
