1. Social Security card

Complete the application prior to arriving at the Social Security office location: 3500 N. Campbell Ave. (northeast corner Campbell & Prince). Office is open 9AM-4PM daily (no weekends). Bring all entry documents with you including Visa, I-94, IAP-66, etc. You may phone 1-800-772-1213 the following day to obtain your number. Obtain a receipt.

2. Dept. Payroll Office

See Teresa Foster, Payroll Representative (BSW 362A). Bring your social security receipt, Curriculum Vitae (if applicable), new residence address and phone number.

3. University Payroll Office

University Services, 888 N. Euclid, Room 402/Customer Service will process the necessary tax forms. Tel: 520-626-3108. Bring your passport and visa. For F-1 visas, bring your I-94 and I-20 forms. For J-1 visas, bring your IAP-66 and I-94 forms.

4. Identification Card (Cat Card)

Must be obtained at CAT CARD office located in the Student Union, room 278. Tel: 626-9162. Bring Recruitment Hiring Form (RHF) with you (obtain from Payroll Rep) and photo ID.

5. Health Insurance

See Teresa Foster, Payroll Rep (BSW 362A), for benefit information if your questions are not answered during Orientation.

6. International Faculty
Research Scholar Office

Visit this office at 915 N. Tyndall Avenue (north of Marriott Hotel) to process your visitation information. Bring your passport, IAP-66 and I-94 forms. Tel: 520-626-6289

7. Keys/Keycard

See Teresa Foster (BSW 362A). You will need to show her your CAT CARD so she can process your keycard to get your keys.

8. Orientation

University Services, 888 N. Euclid, Room 214. A mandatory time will be arranged for you by Teresa Foster, Payroll Rep (BSW 362A). Please bring your employee orientation notice and RHF which Teresa gave you during your first visit.

9. E-mail account

Log on to https://account.arizona.edu/ to create your account. If you are a student, your Social Security number is required. If you are faculty/staff, your Employee ID number is required. For special cases, i.e., visiting scholars, adjunct or postdocs, you will need to visit the Computer Center, Room 204 (SW corner of Mountain & Speedway). Bring paperwork indicating how long you will be here (expiration date required). For more information call CCIT at 520-621-2985. 13. Photocopies

To obtain code to use department photocopies, see Vince Leon, BSW 362B (Tel: 520-621-4951). You will need an account number to charge copies.

14. Parking permits

Parking and Transportation, University Services Building, 888 N. Euclid Avenue, main lobby. Tel: 520-621-7275. Several kinds of permits, some have waiting lists. Discuss with dept. prior to your arrival.

6. Paychecks

17. Risk Management

You will eventually need to take a mandatory Lab Safety Training course. Call Risk Management at 621-1790 to register for this course.
